Chronic Condition Self-Management Training and Support
Chronic Condition Self-Management Training and Support assists individuals with chronic medical conditions to increase their independence in self-management through 1-on-1 training and support. Support is offered for all levels of cognitive ability.
This program is aimed primarily at children, teens, and young adults but can be provided for any age.
Examples of individuals who might benefit from this program are:
a teenager with diabetes that doesn’t monitor his blood sugar or take insulin appropriately,
a young adult who stops taking her medications because she is embarrassed by the side effects,
a child that refuses to wear a CPAP as prescribed,
a teen with cystic fibrosis that skips chest physiotherapy because he want to hang out with his friends instead.
This program is available face-to-face, in the client’s home or community, in Central Indiana (Marion and surrounding counties) and virtual state wide.
This program does not offer medical advice and is geared at addressing behavioral barriers to treatments managed by the individual’s medical team.
