Concierge Care Coordination

Concierge Care Coordination provides expert and individualized direct assistance navigating systems and accessing resources for a flat monthly fee. Unlike current programs provided via waiver and wrap around programs, our concierge care coordination services are supported by a masters level clinician with extensive experience with resources, case management tasks, and care coordination between schools, medical settings, mental health settings, waiver services, and community resources.
This program is private pay only. Telehealth is provided throughout Indiana. Face-to-face is available in Central Indiana if needed.
Collaborative Journeys LLC recognizes that many individuals who need this the most cannot afford a monthly fee and is proud to offer a free 3 month service for free to individuals and families in need enrolling at the start of each month. This is available on a first come first serve basis. Please do not apply for this unless there is financial need. This offer uses the “honor system” and trusts that individuals who do not need the grant will not apply and has no income criteria.
The difference
Many services offer case management as part of their programming, but frequently case managers are overburdened and have limited knowledge of working between systems, funding sources, and community resources. At Collaborative Journeys our Concierge Care Coordinator is a masters level clinician with an additional professional certificate in case management who works between a variety of systems and settings to meet your individual needs including lesser known resources and assistance with applications for difficult to obtain services.

By cutting out funding source and agency requirements for case management services (such as arbitrary meetings and creating goals to check a required box versus what your family actually wants) we get you what YOU need with no additional barriers or obstacles.

At Collaborative Journeys we understand that sometimes things become more complicated than initially anticipated and do not limit the services we provide to individuals originally assessed to be Basic Level just because there is a temporary increase in intensity. Individuals assessed as needing Basic Level of services who end up needing more will continue at the Basic Level fee for six months.

Basic Level
This level is appropriate for individuals who expect needing weekly assistance between 2-3 systems (such as schools, waiver services, mental health services, etc) and monthly face-to-face contact or physical presence at meetings, appointments, and/or conferences.
$150/Month
Complex Level
This level is appropriate for individuals with complex needs who anticipate needing assistance navigating 3 or more systems multiple times a week with 2 or more anticipated conferences, appointments, or face-to-face meetings a month, in addition to frequent phone/email assistance.
