How do I sign up for services?

If you are interested in services please fill out an interest form under the “inquire about services tab.” We will review your request and send you profiles of our clinicians who provide the service you are interested in and our available for you to choose from. You can expect a response in 2-3 business days.

How are services funded and what is the cost?

We currently accept private pay only. Many Tims you can find additional grants to help cover service not covered by your insurance. We are happy to provide you with information about grants we are familiar with.

The cost varies based on the service and the provider you choose. Families will be sent profiles of all the providers who are available with their pricing to choose from. Some services may not have multiple providers or may have providers on a waitlist depending on service and location.

We are not an Indiana Medicaid Waiver agency, but some of our providers do provide waiver services, such as behavioral management and PAC, through other companies and are able to share this information with families, and provide services through their other forms of employment if agreed upon.

Where do services take place?

Services take place at the client’s home, community, or virtually depending on the service you choose and your chosen provider’s availability and service structure. Some providers only do virtual, some only face-to-face, and some will do both. You will receive information about all the available providers and their meeting options in order to make the best choice.

I have a unique service need and can’t find it anywhere. Can you help?

Yes! . Please fill out a request on the “Inquire About Services” page with information about what service you would like offered. After gathering any additional information the request will be sent out to all our clinicians who would be qualified to provide the service and you will be sent their profiles to discuss and design it further.

I have a current provider who does not accept ESA or direct private pay. Can you hire them to provide me with services?

Maybe. Collaborative Journeys, LLC operates utilizing contractors to ensure professional autonomy and maximize reimbursement rates for providers. If your provider is interested in contracting with us they can email Emily Beerbower at ebeerbower@collaborativejourneysllc.com
